New Workers’ Compensation Reporting Forms Required!
The North Carolina Industrial Commission has recently changed some of their workers’ compensation reporting forms. The new forms are required on claims occurring on and after August 1, 2008.
The form change having the most significant impact on employers is the revised Form 19, Employer’s First Report of Injury. This form is to be completed any time a work related injury is reported. A copy of this form is included with this newsletter, and it can be accessed at the Industrial Commission’s web site at: http://www.comp.state.nc.us/ncic/pages/form19.pdf
The Form 18, Employee’s Report of Injury, has also been revised and can be downloaded from the Industrial Commission’s website as well. http://www.comp.state.nc.us/ncic/pages/form18.pdf
